Most people pay no Part A premium. Part D deductibles vary, up to $615 in 2026. These are national reference figures—not a quote for your plan.
The Part D donut hole is gone
The old coverage-gap phase ended in 2025. In 2026, the Part D out-of-pocket threshold is $2,100 for covered drugs. Premiums and drugs outside Part D coverage do not count toward that limit. Check your plan’s formulary and pharmacy costs.
Know which number applies to you
Higher-income beneficiaries may pay an income-related adjustment for Parts B and D. Medicare Advantage, drug-plan and Medigap premiums vary. Original Medicare alone has no annual out-of-pocket ceiling for Part A and Part B care.
Spreading payments is different from saving
The Medicare Prescription Payment Plan can spread covered out-of-pocket drug expenses across the calendar year. It changes when you pay, not the total price. If affordability is the issue, explore Extra Help and other assistance.
A question worth asking
Is the Part A deductible paid only once each year?
No. It applies to each inpatient hospital benefit period. More than one benefit period can occur in a year.
